Volunteer Application

Dear Applicant:

Thank you for your interest in the Eastchester Volunteer Ambulance Corps, Inc. (EVAC). EVAC is the sole emergency ambulance provider for the Town of Eastchester and the Villages of Tuckahoe and Bronxville. The mission of EVAC is to provide state of the art, high quality pre-hospital emergency medical care to the community, to preserve life, and promote the health and safety of the citizens, residents and visitors for our community. As a volunteer ambulance service that provides Basic and Advanced Life Support, EVAC delivers care that exceeds national standards of pre-hospital emergency medical care.

Our volunteer membership application is found below. Please complete this application in its entirety, along with uploading copies of the required documents and certifications (CPR, EMT, Driver’s License, etc.). Once we receive your completed application, we will contact you with the next step on becoming a probationary volunteer member. Your application must be submitted by the last day of the prior month in order to be considered for the next month’s membership meeting. (Example: No later than January 31st for February’s membership meeting.)

EVAC offers flexible hours that can be coordinated with your schedule. On-site training classes are constantly offered by EVAC, both to its membership and to the community. No medical experience other than a valid CPR card is necessary. All uniform items (pants, uniform shirt, and winter jacket) are provided with a required $200.00 cash deposit ONLY which is due at the time you are accepted as a probationary member. This deposit is refundable at the successful end of your 12-month probationary period.

  1. If you were a previous member of another EMS or Fire organization, a letter of satisfactory discharge from that agency is required.

  2. Driver’s license 

  3. Birth certificate OR passport

  4. Valid certifications (CPR, EMT Cards, etc.)
